About the new service

We are creating one integrated specialist learning disability service to offer better, more streamlined and timely support to people with learning disabilities in Sheffield, replacing three teams which were previously running separately.

The new Specialist Community Learning Disabilities service (SCLD) will offer both standard and enhanced pathways which will be provided in a community setting, close to people’s homes.

The standard offer includes core aspects of care which support day to day living for people with learning disabilities.

The enhanced pathway will provide a high priority response to cases that require more intensive support, with the aim of avoiding hospital admissions for people with learning disabilities.

Access to the service is being streamlined, with one entry route for every referral or query. Anybody can complete a referral, either by filling in a form or by calling the service. These referrals will be seen within the daily central hub, where a multidisciplinary team will assess and allocate these onto the most appropriate care pathway.

The service will have bases at both Grenoside Grange and Firshill Rise.

The service’s capacity will be constantly checked to make sure that we are always able to respond to offer support in a timely way to people.

The SCLD will reduce waiting times, provide access based on a person’s specific needs, and improve contact between a named person at the service, service users and their families or carers.

To achieve this new model, we will be increasing the number of staff in every discipline. We will also be offering development opportunities to current staff members to help them be fully equipped to provide the best care possible.

We have put this model together in coproduction with people with lived experience of using learning disability services, carers, SHSC staff and wider NHS and social care experts.

We plan to start working in these ways from this winter, with these being fully up and running by mid-2025.

Senior nurse

This is a varied role where you will use all your skills and can explore the specialisms that interest you. You will be an advocate for some of the most vulnerable people in our society. Work side by side with our patients and their families and carers to enrich their lives.

As a large team of a range of professions we work together, learn from each other and combine our expertise. We assess cases and make sure our patients get the tailored and specific support that will help them the most.

This is a varied role where you will work side by side with our patients and their families, carers and Sheffield’s great services to enrich their lives. You will have development opportunities tailored to you, and will be a central part of helping the service to offer the best support to those we care for.

Nurse associate

You will be at the forefront of developing this role in learning disability services, helping to shape the role to make it the most impactful it can be for the people we care for. In this fast-paced, varied role you will work as an equal partner in our cutting edge service where we use the most specialist, evidence-based and person-centred model available.

Occupational therapists and allied health professionals

No day will be the same as you offer specialist assessments and interventions to our patients. You will be an advocate for them and work to boost their independence alongside their families, carers and Sheffield’s great services. You will be learning every day and developing in a way which is tailored to you, in a team which values and nurtures our occupational therapists and allied health professionals.

You will be using your varied skillset in different ways everyday to make a huge difference to the people we support. Help our patients with communication and Dysphagia while also running autism assessments, all within a supportive and tight-knit unit of a team.

You will tailor plans and advice to offer the best support for the people we care for. You will be acting in the best interests of our patients based on holistic assessments that will make a real difference.
