Eligibility
- The Intermediate Care Mental Health Team' service users are under care of Sheffield Teaching Hospitals Intermediate Care services (Rehabilitation/Therapy)
- We provide mental health assessment, interventions, advice and signposting to adults at home who are receiving rehabilitation support for physical health conditions
- We accept referrals for anyone aged 18 and over who is receiving support or treatment from the therapy teams specified:
- Active Recovery
- Stroke Pathway Assessment Rehabilitation Centre (SPARC)
- Intermediate Care Beds
- Integrated Care Team - Therapy (ICT-T)
- Service users must have a rehabilitation need for a physical health condition and need extra support with their mental health to facilitate engagement with their rehabilitation
- Willingness to engage and want to use the service
- For service users that are not already open to a secondary mental health service and in some cases primary mental health support
- The team follow Nice Guidelines, so service users usually have at least a moderate mental health need
- We work with people with a range of mental health needs including low mood, depression, anxiety, confusion and memory loss. This may also include giving advice and support for carers, supporters or family, and signposting to appropriate support services.
Referral process
Referrals can be made using SystmOne and must have the consent of the individual prior to referral.
Geographical population covered
- People with a Sheffield GP open to the therapy intermediate care teams.
