The Intermediate Care Mental Health Team provides mental health assessment, interventions, advice and signposting to adults who are receiving support for a physical health conditions from the following services:
- Active Recovery
- Stroke Pathway Assessment Rehabilitation Centre (SPARC)
- Intermediate Care Beds
- Integrated Care Team - Therapy (ICT-T)
We work with people with a range of mental health needs including low mood, depression, anxiety, confusion and memory loss. We will support you to make decisions and develop a plan with you.
We also give advice to professionals to help them better support people with their mental health.
How is the service accessed?
We accept referrals for anyone aged 18 and over who is receiving support or treatment from the following teams:
- Integrated Care Team - Therapy (ICT-T)
- Intermediate Care Beds
- Active Recovery
- Liaison Psychiatry
- Stroke Pathway Assessment Rehabilitation Centre (SPARC)
You must have a rehabilitation need for a physical health condition and need extra support with your mental health to help you better engage with your rehabilitation.
Referrals can be made using SystmOne and must have the consent of the individual prior to referral.
