The Assertive Outreach Team provides care for people who have accessed services in the past but the services have been unable to support them.

We provide support in your home, at one of our clinics or in your local community, working flexibility with you to support your mental wellbeing.

When we first meet with you we will carry an assessment of your needs and decide how we can best support you.

We can provide support for things such as access to psychological support or other services, carer or family support and medication reviews.

Our aim to best improve your physical and mental wellbeing, working flexibly with you to support your own personal needs.

Who will support me?

Throughout your time with us, you will have a named member of staff who will co-ordinate your care and provide support.

Depending on what care you need you may be supported by different people such as mental health nurses, occupational therapists, social workers or psychiatrists.

How is the service accessed?

We accept referrals from the Community Mental Health Teams (North and South) based in Sheffield and other NHS assertive outreach services in our region.
