The selection criteria for the service are applicable to:

• Adults over the age of 18;
• People with a variety of needs including mental health problems, personality disorders or mild learning disabilities;
• People who are able to live independently with support.

We take referrals from:

• Acute wards;
• Psychiatric Intensive Care Unit;
• Forensic services;
• SHSC In-patient Rehabilitation and Recovery Services;
• Locked rehabilitation hospitals;
• Community mental health teams;
• Sheffield Assertive Outreach Team

• The registered populations of NHS Sheffield Clinical Commissioning Group;
• People without settled accommodation who are resident in Sheffield.
