Eligibility

The selection criteria for Birch Avenue are applicable to:

• Adults irrespective of age, gender, ethnicity, sexuality, culture or physical abilities providing the service can meet their assessed need
• All adults with a nursing need and diagnosis of dementia illness with enhanced complex needs.

Geographical population covered

• The registered populations of NHS Sheffield Clinical Commissioning Group
• People without settled accommodation who are resident in Sheffield
• People from out of town with agreed funding arrangement
