The Older Adult Community Mental Health Team supports people aged 65 and over who are experiencing mental health difficulties. 

We support people who are experiencing problems such as psychosis, mood disturbance and memory problems, who have related behavioural or psychologial symptoms. 

The team covers the entire city and is organised into four geographical areas; North, West, South East and South West. We provide support in each of these local communities, including home visits or visits to care homes. 

When you or a loved one first come into contact with the service we will arrange to carry out a comprehensive assessment of your physical and mental health needs. We will also look at your social situation and personal history. 

From there we aim to provide a diagnosis and agree a plan to help address your needs. Your care may include speaking to a psychiatrist who could prescribe you medication depending on your diagnosis. You may also be referred for psychological therapy, occupational therapy or we may recommend you attend a group session which can address issues such as anxiety and depression. 

Our team is made up from Support Workers, Nurses, Occupational Therapists, Administrators, Psychiatrists and Psychologists. We also have students as part of our staff team.

Accreditation

In May 2021 we were delighted to find out that we had been successfully accredited with the Royal College of Psychiatry

The accreditation programme works with staff to assure and improve the quality of community mental health services for people with mental health problems and their carers.

How do I access the service?

The majority of the referrals the team receives come from GPs within Sheffield, but we also accept referrals from other Trust services. 

All referrals to the Older Adult Community Mental Health Team will be triaged to see which of the Trust’s services can best support your needs.

This may include onward referral to our Older Adults Home Treatment Team or Memory Service

We also support people aged under 65 under certain circumstances. Before you make a referral please make sure you have taken a look at our Older Adults Mental Health Protocol and our Dementia Protocol.

The Coronavirus pandemic has brought a lot of changes to our lives, and it has also meant we've had to make some temporary changes to the way we provide our support.
