Community Mental Health Teams: Adults

  The service is based at four bases across the city but is registered with the Care Quality Commission at Trust Headquarters, Fulwood House. The four adult Community Mental Health Teams (CMHTs) in Sheffield provide a service for people with mental health problems.  The types of condition treated include psychosis, schizophrenia, bipolar disorder, severe depression, anxiety, personality disorder and Post Traumatic Stress Disorder (PTSD). The Community Mental Health Teams each comprise an Access Team (for assessment and short term interventions), a Recovery Team (for long term interventions), an Early Intervention Service (for young people with suspected first episode psychosis) and a Home Treatment Team (for short term intensive interventions). Operating hours

Access Team: Monday to Friday 9am-5pm Recovery Team: Monday to Friday 9am-5pm Early Intervention Service: Monday to Friday 9am-5pm Home Treatment Team: 7 days a week 8am-6pm Outside of these hours emergency support is available from the Out of Hours Team.

How is the service accessed?

Referrals are accepted from GPs or other clinical teams within Sheffield Health & Social Care NHS Foundation Trust.  Referrals may also be accepted from other health or social care professionals, housing, voluntary sector, relatives, and by self-referral.
